I am aware of the nature of this activity and hereby assume full responsibility for the participant listed above to engage in such activities. I acknowledge that, while not common, any activity involving height and motion (including but not limited to bounce houses, tumbling, and stunting) carries a risk of injury ranging from minor (such as bruises and sprains) to more serious or even catastrophic injuries.

I agree that I will not hold Topflight Athletics, its owners, employees, or representatives liable in the event of accident or injury resulting from participation. I understand that this completed form must be on file with Topflight Athletics prior to participation in any program.

If at any time I have a question or concern regarding my child’s safety or the intent of the program, I will immediately contact the owner, manager, or instructor.

I further grant permission for the participant listed above to be photographed and/or videotaped for publicity and advertising purposes only.
